Paul Mendis, MD, Neighborhood Health Plan, 253 Summer Street, Boston, MA 02210, 617/428-7426, Paul_Mendis@nhp.org

The New England Regional Public Health and Managed Care Collaboration was instituted to harness the strengths and knowledge of public health and managed care in New England in developing and applying preventive and population-based clinical and community-based health interventions to improve the health status of New Englanders and their communities. The Collaboration consists of the six New England public health commissioners, New England's managed care leaders and the U.S. Department of Health and Human Services (New England Region), as well as the New England Coalition for Disease Prevention and Health Promotion (NECON) and the Heller School at Brandeis University. The collaboration's four workgroups focus on the areas of adult immunization, diabetes, pediatric asthma and tobacco control and cessation. This presentation, which will discuss the diabetes workgroup, will (1) illustrate how public health and managed care developed consensus on best practices in the care of diabetes, (2) share strategies on implementation of best practices across the region and (3) consider both in the context of current racial and ethnic disparities in the diagnosis, treatment and health outcomes/sequelae of diabetes.

Learning Objectives: 1. The presentation will illustrate how public health and managed care reached consensus on best practices in the care of diabetes. 2. The presentation will share strategies that support implementation of best practices with the public and providers across the region
