Assessing the health related needs of youth with chronic health conditions and disabilities

This presentation will describe the subjective health-related experiences and priorities related to self-management of health care and health promotion of teens with chronic health conditions and physical disabilities in North Carolina. The findings, through a series of focus groups and health surveys conducted with teens and young adults having chronic health conditions and physical disabilities and parents of teens with chronic health conditions and disabilities, will be discussed. The health experiences and priorities of teens, young adults, and parents will be compared. Emphasis will be on identified barriers and recommendations to promote receipt of basic preventive and health promotion services for youth with chronic health conditions and disabilties. In addition, planned NC Office on Disability and Health initiatives in the areas of policy development, service delivery, and consumer/provider collaboration will be discussed.

Learning Objectives: At the conclusion of the session, the participant will be able to: 1. Idendify barriers to basic preventive and health promotion services for youth with disabilities. 2. Articulate recommendations to promote receipt of basic preventive and health promotion services for youth with disabilities. 3. Describe potential policy and service delivery options that support the health related needs of youth with disabilities
