Gerald L. Stevens, DC , MS, MPH, New York Chiropractic College, 4974 Transit Rd., Depew, NY 14043, 716-685-9631, gstevens@nycc.edu

Objective: 1) Develop an evidence based, efficient method to deliver overall wellness in an office setting. 2) Test the tool in a private office setting to determine overall usefulness. Methods: A health and wellness task force was appointed by administration whose members had various abilities to formulate a tool to promote and influence overall wellness to patients. Public health issues were divided amongst members and they were assigned to find free evidence-based websites/resources. The doctor could efficiently use a checklist and associated websites to help educate patients to address their health issues. Results: After input from various staff members a one-page checklist with evidence based free websites was formulated. The next stage of this study is to test this tool in a private office setting and obtain feedback via a survey. Conclusion: To be determined when data analysis is complete.
