Brief technical descriptions of key platforms and software applications will be presented. Mr. Robinette will then describe specific IT constructs now in use, operational problems and limitations, progress (ONCHIT/NHIN) toward interoperability, and limitations confronted in specific secondary public health uses of the data (VAERS/Biosense). These limitations include: scope and validity of data; timely access to data; and complex organizational linkages required to support systems.

Mr. Robinette will then describe how each of the issues above might be impacted by “inverting the pyramid.” The discussion will focus on possible positive impacts in two key areas: EHR/PHR as a vehicle for empowering the consumer; and potential effects on PH monitoring and surveillance activities.
