Evidence-based Policy and Practice on Homelessness
This session addresses some of the complexities of providing services to homeless mentally ill and those with other disabilities. In this session presenters will discuss a team centered approach of integration of health care. In this session presenters will also discuss the Assertive Community Treatment (ACT) model for use among hard to treat homeless people with severe mental illness.
Learning Objectives: 1. Describe impact of living in shelters on health of adults with disabilities 2. Identify barriers for homeless accessing health care 3. Identify concepts to creating an integrated system of care for homeless 4. Define Assertive Community Treatment ACT for homeless mentally ill
