"A Practice of Social Medicine,” both the 1948 book by Sidney Kark and his later classic article, describe the pioneering work and innovations of his Pholela community health center, the first in South Africa. They describe the merger of primary care with public health interventions including the use of epidemiologic investigation and surveillance to guide both clinical practice and targeted interventions in nutrition, environmental change and health education. Phoela, in turn, was the template for the first American community health center, the Delta Health Center in Mississippi, in the 1960s, which added community empowerment, employment, educational opportunity and a broader focus on the social determinants of health to the original model.
