On the Move is a multi-level health initiative targeting nutrition, physical activity and wellness in a rural Appalachian county in Kentucky. Its mission is to develop and implement, with community participation and buy-in, programs and activities that will help create a healthier lifestyle and environment for a traditionally underserved area. This presentation will describe both the successes and challenges of nutrition and physical activity program creation when confronting geographical, social and economic barriers. The target population ranks in the bottom third of the state according to The Health of Kentucky, a 2007 statewide county assessment by the Kentucky Institute of Medicine. Most relevant to this initiative , they have higher rates of overweight and obesity than the rest of the state, and the highest level of inactivity in the state. The presentation will highlight On the Move's program successes at the community level, including a weight loss and nutrition program, environmental improvements such as improved accessibility to walking and fitness trails, and collaboration with school districts in promoting and delivering walking and activity programs. Participants will also gain knowledge of the lessons learned and challenges encountered when trying change health-related behavior in adults, including political barriers, embedded health myths, and the financial constraints facing a proud but economically distressed community.
