4384.0 Childhood Obesity Prevention in New York City Community Health Centers: Best Practices and Lessons Learned

The Community Health Care Association of New York State (CHCANYS) formed the New York City (NYC) Obesity Prevention and Management Consortium to address childhood overweight/obesity in community health centers (CHCs). Over the span of 3 years, CHCANYS coordinated learning teams at 11 participating CHCs. During this process, teams were provided with training and technical assistance on the framework of the Chronic Care Model, Model for Improvement, recommendations from the Expert Committee on Childhood Obesity and the evidence-based We Can! curriculum. CHCANYS also partnered with the Children’s Museum of Manhattan to train teams on unique and interactive ways to educate parents and children about healthy eating and physical activity. Based on successful results the consortium, the U.S. Surgeon General awarded the Consortium with the Healthy Youth for a Healthy Future Champion Award. CHCANYS is confident that an array of organizations interested in childhood overweight and obesity prevention can apply a similar framework to make positive sustainable changes in addressing childhood overweight and obesity.
Session Objectives: Identify processes to increase documentation of Body Mass Index (BMI) and Healthy Weight Classification within an Electronic Medical Record. Utilize interventions with overweight or obese children and their caretakers for talking about obesity with families. Develop strategies for engaging community based organizations as partners to combat childhood obesity.
