274674 Using Healthy People 2020 across the Life Span to drive local health department and community planning efforts

Since 2005, the Florida Department of Health at St. John's County Health Department (FDOH-SJCHD)has been using both key health indicators, and targets as a major source of input in our Local Health Department strategic planning process, as well as in the development of our Community Health Assessments (CHA) and Community Health Improvement Plans (CHIP). In 2005, FDOH-SJCHD began using an Organizational Balanced Scorecard process for identifying strategic focus areas, objectives and performance indicators, and targets. The current version of this Balanced Scorecard is comprised of four focus areas: 1.0 Promote & Protect Health and Quality of Life 2.0 Business/ Finance Excellence 3.0 Customer and Partnership Engagement 4.0 Employee Engagement/ Accountability The objectives set forth under the Promote and Protect Health and Quality of Life focus area are strongly aligned with various Healthy People 2020 indicators and targets that reflect the life span of our community's residents and visitors. Similarly, in 2005, FDOH-SJCHD completed its first CHA, using the MAPP process (Mobilizing for Action through Planning and Partnerships), with follow up Assessments completed in 2008 and 2011. In the 2011 MAPP cycle, SJCHD's CHA/CHIP was enhanced to include a Community Balanced Scorecard that depicted a four-tiered perspective of objectives to be addressed by the CHIP. They include the following: 1. Community Health Status 2. Community Implementation 3. Community Learning and Planning 4. Community Assets

The objectives established for the Community Health Status perspective are strongly linked to various indicators and targets set by Healthy People 2020 that reflect the life span of our community's residents and visitors.
