274711 Incorporating Healthy People 2020 into Local Work

RiverStone Health, the health department in Yellowstone County Montana, like many communities across the United States recently completed a Community Health Assessment. The completion of this health assessment provided an opportunity for Yellowstone County to compare ourselves to the national data and to identify areas in which Healthy People 2020 measures were not achieved. Utilizing the Healthy People 2020 measures as goals a local health coalition has set out to achieve optimal health for all residents, of all ages, in Yellowstone County. This presenter will demonstrate how the Healthy People 2020 measures can be put into action with a local community coalition.
