The Affordable Care Act (ACA) and other federal initiatives are fostering the emergence of a coherent vision for chronic disease prevention that has never before existed in the United States. The presentation will also illustrate an individually targeted strategy that helps patients obtain health insurance coverage using community health workers and an institutional policy to facilitate healthy choices for patients, employees, and the broader community served by the health system.
