National Diabetes Education Program -Primary Care Team Approach to Diabetes Utilizing the Health Professions of Podiatry, Pharmacy, Optometry, and Dentistry

The National Diabetes Education Work Group, PPOD (Pharmacy, Podiatry, Optometry, Dentistry Work Group), was established to promote the principles of NDEP by utilizing Pharmacy, Podiatry, Optometry, and Dentistry organizations and providers to increase awareness among individuals with diabetes and their health care providers of the need for and access to quality care by enhancing timely referrals among these four primary health care professions.

Pharmacy, Podiatry, Optometry, and Dentistry (PPOD) are underutilized channels for the delivery of health messages to people with diabetes. This presentation will review the CDC-PPOD Work Group mission, objectives, and strategies to 1) utilize PPOD channels to disseminate diabetes messages, 2) increase knowledge base of primary care providers regarding timely referrals among these four primary care providers, 3) increase access of diabetes care through collaboration among PPOD organizations and providers, and inform policymakers and health care purchasers about the benefits of integrated diabetes care.

Learning Objectives: Define role of pharmacy, podiatry, optometry and dentistry in a primary care team approach to diabetes. Discuss enhanced referrals among these four primary care providers in order to facilitate better control of diabetes and the prevention of complications. Identify key messages pharmacists, podiatrists, optometrists, and dentists can deliver to their patients with diabetes to enhance control of diabetes.
