Palliative care needs of medically-underserved persons with HIV/AIDS in an urban inner city

Montefiore medical Center is implementing an augmented palliative care model to deliver a range of services to seriously-ill HIV-infected patients in the New York metropolitan area. This demonstration program is supported by a grant from HRSA/SPNS. In addition to a physician, psychologist, nurse, social worker, and chaplain, the multi-disciplinary care team includes an addictions specialist and outreach worker. Care is delivered to patients throughout the hospital's ambulatory care network, the HIV/AIDS center, and in the nursing and substance abuse facilities. This presentation will describe the range, focus and challenges in delivering palliative care to individuals with HIV/AIDS who are uninsured poor, substance abusers and homeless.

Learning Objectives: At the completion of this session, participants will be able to: understand innovative approaches to delivering palliative care services to hard-to-reach populations, appreciate the challenges of delivering palliative care services to individuals with HIV/AIDS, and comprehend patient-centered end-of-life care as it relates to HIV/AIDS.
