Providing palliative care to the homeless with HIV/AIDS: End of life residence approach

Catholic Community Services is addressing the challenge of ensuring that homeless individuals has access to palliative care services by establishing a community-based residence. The residence, in an urban area of New Jersey, will serve homeless, multiply-diagnosed clients who are terminally-ill with HIV/AIDS. In this demonstration program, funded by HRSA/SPNS, Catholic Community Services will provide the homeless with a residence and home health aides who perform needed personal care services. It is then possible to establish links with local hospice and palliative care providers to deliver services to the residents in the residence. The residence-based care team is multi-disciplinary, including a social worker, nurse, and health aides. The presentation will discuss the project implementation experiences.

Learning Objectives: At the completion of this session, participants will be able to: understand innovative approaches to delivering palliative care services to hard-to-reach populations, appreciate the challenges of delivering palliative care services to individuals with HIV/AIDS, and comprehend patient-centered end-of-life care as it relates to HIV/AIDS.
