Lower extremity diabetes disease management: results of population based screening and prevention in a managed care population

This presentation will provide data from a 30 month follow-up of a population based screening program designed to identify high-risk patients and provide preventive care. 1600 diabetics were screened and treated from a managed care population. The authors will describe the incidence of diabetes related foot complications and effectiveness of prevention practices.

Learning Objectives: The objective is present data from long term follow up of a population based screening program in a managed care enviroment.
