Non-Hodgkin's lymphoma in a cohort of HIV/AIDS patients in Puerto Rico

Background: The immune dysfunction caused by the HIV is related with significant increment of malignant diseases. Even thought close association between AIDS and non-Hodgkin's lymphoma (NHL) is known, few study had evaluated these conditions in Puerto Rican patients. Methods: A cross sectional study was performed in a cohort of 2,843 HIV-infected patients followed in the Retrovirus Research Center at Bayamón, Puerto Rico, until December 2000. NHL prevalence was determinate and differences between NHL groups were evaluated with the help of Fisher and ANOVA tests. Results: The NHL prevalence was 0.9 % in the whole HIV infected cohort and 1.6 % in the AIDS patients. Nine(36%) were AIDS-defining (AIDS-d) NHL and 16 (64%) were non-AIDS-d NHL. Both groups were similar in gender distribution (78% vs. 81% male) and in mean age at lymphoma diagnosis (42.7 vs. 41.8 years). The median CD4+T cell count at NHL diagnosis was of 80/mm3 in the AIDS-d NHL group and 108/mm3 in the other group. Antecedents of injecting drug use was higher in AIDS-d NHL patients (56% vs. 19%) and Homo-Bisexual contact was higher in non-AIDS-d NHL patients (14% vs. 62%). Death rate in the first year after the NHL was of 67% in the AIDS-d group and 38% in the non-AIDS-d group. Conclusions: The NHL prevalence in this HIV/AIDS cohort was lower than in previous studies (4% or 9%). However the study detected a highly compromised immune system in these NHL patients that could explain their poor prognosis, especially in the AIDS-d cases.
