Focusing on what works: A study of high performing health facilities in Kenya

Some clinics work well even in less-than-ideal conditions. We studied what is working well, asking the question: Why are some health facilities resilient and able to succeed with similar resources and constraints while others are not? A sample of high- and low- performing facilities in Kenya was examined for factors contributing to resiliency and overall high performance.

Most studies have focused on identifying problems and barriers preventing sites from providing quality services. This phased study was conducted within the framework of examining assets rather than deficits. This will allow modeling factors of high performance as a means to help low or average performers make sustainable improvements.

Through observations, interviews and focus groups of personel, clients and community members at 25 clinics it was found that the primary factors contributing to a site’s resiliency and high performance were: (1) Community involvement (e.g. Community members on management teams or involved in maintaining the clinic, feedback mechanisms, staff working in the community (2)Financial management (e.g. cost-recovery mechanisms and autonomy for spending funds) (3)Motivated and well-trained staff (e.g. “immediate changes” or “quick successes” was key to motivation, training, tea breaks, departmental awards, time off) (4) Leadership (e.g. leads by example, open communication, ability to delegate work/duties, honesty)

Detailed findings and strategies for improving and maintaining performance validated in this study will be presented.
