System-wide file review in a chiropractic educational system of clinics

Assessment of faculty is as important as student assessment. Multiple assessment methods provide a better understanding of the tasks encompassing a faculty clinician’s duties. A large chiropractic college consisting of multiple clinics created a system-wide file review procedure. The purpose of the file review process was tri-fold measuring: compliance issues and quality control of current files, documentation for the patient concerning patient care issues, and third-party pay issues pertaining to reimbursement from insurance companies and enhancing the professional image with outside agencies. A workgroup tasked with designing a procedural model for file review appointed four permanent members and two rotating clinicians to serve for a one-trimester period. Each member reviewed files for 1-2 clinicians selected at random. A checklist with the following headings: file organization/legibility, history review, physical/orthoneuro examination, special studies, impression/management plan, daily progress notes, overall documentation, overall impression, and recommendations was utilized to demonstrate medical necessity. A written report template mirroring the statements in the checklist was created. Each reviewer randomly selected 2-4 active patient files. Clinic compliance guidelines require that daily charges be compared with daily progress notes. A report was generated using a report template and was forwarded to the clinic director who discussed the report with the clinician. File review information was utilized as a part of the annual performance review of the clinician. Faculty being audited assess the file review process by means of a questionnaire. The process was reviewed at the one-year point.
