Medicare Primary and Consumer Directed Care Demonstration: Health Related Quality of Life Outcomes

During the past decade health related quality of life (HRQOL) has become a construct that is regularly evaluated in health services research.

The aim of this study was to evaluate the HRQOL of 1609 Medicare beneficiaries enrolled in the CMS Demonstration "A Randomized Controlled Trial of Primary and Consumer-Directed Care for People with Chronic Illnesses".

Frail Medicare participants from upstate New York and the mid-Ohio valley (WV/OH) were randomly assigned to one of four groups: 1.A consumer-directed Voucher for home care services, 2. A Primary Care Health Promotion Nurse, 3. Combination of consumer-directed Voucher and Health Promotion Nurse, and 4. A Control group that receives usual community care.

An interviewer-administered self-reported battery of quality of life measures including the SF-36 were used to evaluate the hypothesis that the intervention impacted on quality of life, including functional status and health status.

We will report baseline and 22-month data that tests the hypothesis that better quality of life will be experienced: a. for the Voucher group compared to the Control group; b. for the Health Promotion Nurse group compared to the Control group; c. for the Combination group compared to the Control, Voucher or Health Promotion Nurse groups.
