Churches provide an important basis for programs to improve community health, but often programs are implemented based on the church's opinion of what the community should have rather than on data that document what the community actually needs. This paper will present a model for a church-based community needs assessment strategy that has proved successful in (1) preparing churches for assessment; (2) conducting the assessment; (3) planning and implementing community services; and (4) evaluating the process. The presentation will highlight the community survey instrument, a program for the education of church members, a simple evaluation strategy, and a method for involving the entire church membership in the process.
