Smoking attributable fraction. A matched case-control study to evaluate its impact in cardiovascular heart disease

Learning Objectives:
Introduction.
Coronary heart disease (CHD) is the main cause of mortality in Mexico. Cigarette smoking is a well-known risk factor for this pathology and other diseases, inability and preventable premature death. It's prevalence in our country is higher than 27% and it's unknown its contribution in CHD.
Objective.
To evaluate the impact due to cigarette smoking in coronary heart disease and also to propose a simple and efficient standard methodology in order to estimate the attributable fraction.
Method.
We carried out a case-control study in the Regional Hospital of Mexican Institute of Social Security in Morelos State, from May to June-200- The cases were 80 prevalent and incident patients with CHD. One hundred sixty outpatients were selected randomly matching by sex and age. The attributable fraction was obtained through odds ratio summarized, variance and confidence intervals for matching designs with more than one control for each case.
Results.
Prevalence of cigarette smoking previous to coronary event was 80% compared to 62.1% of controls prevalence. Mantel & Haenszel odds ratio showed a positive association to cigarette smoking with CHD (OR, 6.7; 95%CI, 3.2-13.8). Attributable fraction among exposed individuals was 85% (95%CI, 74-9
- while population attributable fraction was 56% (95%CI, 45-66), in men were 65% (95%CI, 53 -7
- and women were 31% (95%CI 14 - 47).
Conclusions.
Smoking impact on CHD is high. The used method allow us evaluate efficiently the attributable fraction and weighted a mortality participation due to this pathology and may be to detect changes derived of the anti-tobacco programs.
