Increasing access to care through web based techonology: The Fulton Atlanta Community Access Coalition

The city of Atlanta and Fulton County Georgia have a sigificant percentage of uninsured populations. There have been no successful method of tracking these individuals, providing referral and follow up for those who have been identified and no sources of additional funding for direct services they might otherwise have been unable to afford. A web based network of social service providers has demonstrated some success with uinsured populations presenting for these services. No prior linkage has existed however between social service providers and clinical service providers. This paper describes the development of a new coalition of providers in Atlanta and Fulton County that successfully links these referenced providers and increases the diversity of direct service provided to those in need. Further, it describes the unique aspects of this web based system using PC's, wireless hand held devices and outreach workers as the foundation of the approach. It also addresses the issues of HIPPA compliance and setting the framework of fairness among the many partners that include health departments, FQHC's, hospitals and non profit service agancies.
