Systems Integration: Identifying faciliators and barriers to the process: Networking with the community for systems integration

Recently the U.S. Surgeon General (2000) services for older adults are insufficient and fragmented, and are often divided between systems of physical health, mental health, public health and social services. This paper examines the process of systems integration in an effort to identify how to improve mental health interventions for older adults. Facilitators and barriers to effective interventions for effective systems integration will be identified. A model known as the “VISION” model will be used to articulate a process for networking stakeholders within the community for the purpose of building integrated systems of care. A case study illustrating the process in rural southern Illinois will be used to demonstrate how the steps in the model worked.
