Postpartum HIV/AIDS health intervention program (PIP): An innovative public health nursing initiative

There is scant data to define the HIV epidemic among pregnant women and their infants, but from 1998 –2000 over 200 HIV+ women delivered infants in Philadelphia and of those more than 50% had received no prenatal care. The Postpartum Intervention Program (PIP) is a collaborative between La Salle Neighborhood Nursing Center (LNNC) and the Philadelphia Health Management Corporation (PHMC). PIP is a newly designed home visitation model targeting out of care postpartum women and their infants at high risk for HIV exposure and infection in North Philadelphia neighborhoods. The PIP collaborative builds on the unique and shared PHMC and LNNC resources, goals and expertise. PHMC has provided HIV services to people at high risk through prevention outreach, risk reduction education, service coordination and research since 1987. Since 1991, LNNC has provided health education, public health and primary care services to vulnerable urban populations in Philadelphia neighborhoods. LNNC public health nurses (PHNs) link targeted women and their newborns with appropriate health care providers through home visitation services. In addition, PHNs provide counseling services, perform in home HIV testing; emphasize risk reduction behaviors; serve as prevention educators; and make community referrals to promote family well being. This presentation reviews the evolution of PIP and describes client outcomes based on the logic model and outcomes tracking grid. Lessons learned and future work will be shared as the collaborative demonstration project enters its second year of funding.
