Fifty percent of deaths in the United States each year are attributed to cardiovascular disease (CVD). Cigarette smoking, elevated blood pressure and blood cholesterol, physical inactivity and obesity rank among the known risk factors. This project was based on a community assessment using the Anderson and McFarlane Community Assesssment Wheel Model. This model was chosen as it provides a comprehensive view of a community assessment and provided a framework in which to collect data. A convenience sample of community residents completed the Health Risk Appraisal and were measured at pretest and postest for systolic and diostolic blood pressure, weight and BMI. Data was analyzed using T tests for dependent samples. One hundred per cent of the participants recorded a change in behavioral lifestyle patterns due to the educational programs. The HRA risk and target age decreased from pre to post test and the participants were able to identify modifiable and non modifiable risk factors. The programs were found to significantly improve the health of the participants by loss in weight in pounds, decreased BMI, and lower systolic and diastolic blood pressure. The findings suggest that community based cardiovascular health promotion programs can have an effect on the population. Continued programs are necessary in order to reach a larger portion of the rural community.

Learning Objectives: At the conclusion of these health programs, the learners (participants) will be able to
