Predictors of mortality in elderly patients following hip fracture: A prospective evaluation

Purpose: This study was performed to determine predictors of survival time and to examine the role of specific type, number, and severity of co-morbidities on mortality following hip fracture. Methods: Between 1987 and 2001, 1076 patients with a hip fracture were registered in this database. Patients were identified at the time of admission and all data collected prospectively. After discharge, the patients were regularly followed over the phone at 6 month intervals. Data were analyzed using a Cox Proportional Hazard Model. Results: 936 (87%) had complete information on covariates of interest and were included in the analysis. Twenty-six patients (2.6%) died in the hospital while the 1, 2, and 5-year mortality rates were 11%, 16.4% and 33.6% retrospectively. The following variables were shown to be significant predictors of mortality; age, (Hazard ratio (HR)=1.04, p <. 0001, 95% (CI)=1.03-1.06), male gender (HR=1.4, p <. 001, 95% CI=1.2-1.8), a high ASA (HR=1.5, P<. 0001, 95% CI=1.2-1.8), and the presence of major comorbidities, specifically cancer (HR=1.7, P<. 001, 95% CI=1.3-2.3), COPD (HR=1.4, P=.02, 95% CI=1.0-1.9), and peripheral vascular disease (HR=1.2, P <. 01, CI=1.0-1.9). Conclusions: Final model for predicting mortality post hip fracture suggested a significant role for age (per year); male gender, ASA score, and major comorbidities including cancer, COPD, and peripheral vascular disease.
