Empowerment of the older adult involves not only the provision of services but also the collaboration of efforts to strengthen an individual's decision-making ability. Health promotion and disease management strategies can improve outcomes through the development of self-care management, behavior change, and skill building. This paper describes a case study investigation of the health promotion nurse arm of a randomized controlled trial of consumer-directed care of 1609 frail, chronically ill Medicare beneficiaries conducted in NY, WV, and OH. The Health Promotion Nurse endeavors to eliminate or reduce problem health behaviors and expand patient involvement in their own care through the completion of a standardized assessment in the patient's home, employment of a variety of self-care and self-management approaches, joint meetings among the physician, participant, caregiver, and nurse (reimbursed by Medicare waivers), the use of goals and other health promotion strategies, and emphasis on exercise and physical activity. The case studies will be presented to elucidate how the nurse model impacted quality of life, functional status, health status, hospital, emergency department, and nursing home use outcomes. Extensive quantitative and qualitative data collection was conducted over the 24-month intervention period. The study is completed and final results will be presented.
