Chronic pain has been identified as a significant comorbidity in older adults, in general. However, the prevalence of chronic pain in frail older adults and the interplay among pain, function and quality of life is not as well articulated. Data from the two year follow up of the Consumer Directed Care Demonstration Project will be presented to chart the prevalence of pain and its impact on the functioning and quality of life of older adults. In addition, the reciprocal relationship among self-efficacy beliefs, pain and function will be examined to shed light on the moderating role of self efficacy in health behaviors and function. Finally, the impact of consumer directed care models, in particular the activities of the Health Promotion Nurse on pain management will be examined.
