Implementation of an innovative capitated program for people with severe mental illnesses

Baltimore Mental Health Systems, Inc., Baltimore City’s mental health authority, developed a performance contract capitation program to serve “high cost” severely mentally ill patients, with the aim of providing incentives for service providers to develop novel and cost-effective solutions to service delivery problems. This paper describes the implementation of the program from the perspective of the mental health authority and the service providers. Two clinical cohorts will be described: the first consists of state hospital patients discharged to the community after an average hospital stay of more than five years, and the second consists of adults from the community with more than four psychiatric hospitalizations in two years. The implementation of the funding mechanism with its system of bonuses to reward good performance will be described and service providers will discuss the clinical and logistical challenges in implementing the program and the innovations that the capitated funding enabled.
