Institutional Capacity Building: A model for sustainable maternal health training at district hospitals in Indonesia

In Indonesia, one strategy to achieve the Ministry of Health goal of increasing the numbers of births attended by skilled providers is strengthening the skills of midwives through inservice training. Implementing this strategy showed that a strong maternal and newborn healthcare training system required not only well-trained trainers but also quality clinical settings in which providers can observe, learn and become proficient in their skills. To that end, the Maternal and Neonatal Health Program (MNH) has supported the institutional capacity of the National Clinical Training Network (NCTN) to develop an inservice training system that focuses on high quality, clinical practice to achieve competency. NCTN is embedded in the professional organization of obstetricians and has been legitimized through a government decree, making it the sole provider of clinical training in reproductive health. MNH and NCTN are using a performance and quality improvement approach to strengthen a network of MCH service delivery sites so that they can serve as clinical training sites. Activities include involving administrators in the planning process, mentoring hospital midwives on a continual basis, and linking the clinical training sites to the midwifery academies. Results from 18 months of activities indicate that the preparation of high quality clinical training sites requires complex inputs to strengthen skills in clinical decision making, mother-friendly care, proper infection prevention practices, and prevention and management of complications. Once these inputs were in place, however, appropriate learning is then facilitated and skilled providers result.
