Concurrent Validity of Administering the Functional Independence Measure (FIM) by Interview

Objective: To measure the concurrent validity of administering Functional Independence Measure (FIM) by interview.

Design: Concurrent administration of the FIM by a trained interviewer and by a multidisciplinary team.

Setting: Five post-acute rehabilitation facilities in the Baltimore metropolitan area, 2001.

Participants: Twenty-eight community-dwelling, non-demented, elderly hip fracture patients.

Measurements: Within 72 hours of admission to (and again discharge from) the post-acute facilities, both a FIM-certified interviewer and a multidisciplinary team administered the FIM to each participant. The FIM score (range 18-126) is the sum of ratings (range 1-7) of the participant’s independence in performing 18 activities.

Results: At admission, the mean FIM scores assigned by the interviewer and by the team were 84.3 and 80.5, respectively (p<0.001). At discharge, the mean FIM scores were 104.4 and 103.4 (p>0.51), reflecting the participants’ functional recovery during their post-acute rehabilitation. The intra-class correlation between the FIM scores assigned by the interviewer and by the team was 0.74 (95% CI=0.58-0.91) at admission and 0.76 (95% CI=0.60-0.92) at discharge.

Conclusion: FIM scores assigned by a FIM-certified interviewer are valid indicators of the functional status of community-dwelling older people recovering from hip fracture.
