Adding social services to a disease management program

Objective: To describe the results of adding a social services department to a disease management program. Methods: In May of 1998, a social services department was instituted at Corsolutions, Inc. It was chosen as being the most likely needed adjunct service. The number and composition of referrals was examined. Findings: From May 1998 to February of 2002, the department received almost 4,000 referrals (N-3990.) Referrals for financial concerns were 47% of total referrals each month. The CHF population, which had been on program at least 6 months, accounted for 9.4% of all referrals. On average, 8-10% of the company’s high-risk population was referred per year. 12% of referrals were for Crisis intervention. Services provided to patients included assessment, education and linkage with available assistance options, counseling regarding long term planning or financial planning; bridge therapy; counseling to caregivers; crisis response; and referral to oversight agencies. Regular referrals took between 12-14 contacts to process, with crisis referrals being an exception to that number, bringing the overall total to 18 contacts per patient. Discussion: The program was found to be helpful to patients and desired and used by disease manager/RNs. It minimized inappropriate hospitalizations especially those due to inadequate resources/lack of medications, and addressed compliance issues. It also allowed for more appropriate use of professional staff time. Implications: The addition of telehealthcare social work services is a value added, cost effective approach to enhancing patient care which could be instituted in any medical setting with positive outcomes for provider and patient alike.
