Reducing pressure ulcers in long term care facilities: Results of a continuous quality improvement project in New Jersey

A continuous quality improvement (CQI) project to reduce the incidence and prevalence of pressure ulcers involved 13 long-term care facilities in New Jersey. We provided data collection tools for specific elements of initial patient assessment and care plans. We also provided CQI technical assistance through workshops, on site visits and telephone calls. The facilities collected their own data and we provided analytic reports to help them identify areas in need of improvement. Once identified by the facilities, we helped them develop their quality improvement plans, interventions, measurement and remeasurement tools.

This presentation will discuss the results of this two-year project, including reductions achieved in incidence and prevalence of pressure ulcers, major barriers and lessons learned, and successful intervention strategies used by the facilities.
