Evaluation of a nutrition counseling program in Philadelphia Department of Public Health Family Health Centers

The Philadelphia Department of Public Health (PDPH) has implemented the Philadelphia Public Health Nutrition Education Initiative (PPHNEI) with a mission of providing literacy and culturally appropriate nutrition education to improve the overall health and well-being of Philadelphians who are receiving or eligible to receive food stamps. Since July of 2000, nutrition professionals have provided individual nutrition counseling for adult patients in eight PDPH Family Health Centers, where over 93,000 low income Philadelphians receive health care annually. Nutritionists are counseling patients, primarily adults, who present with diabetes, hypertension, cardiovascular disease, obesity, and a host of other medical problems. This study is designed to evaluate the impact of this newly implemented individual nutrition program on the health of PDPH Health Center patients and provide a model for ongoing data management and program evaluation. This project uses a quasi-experimental, pre-intervention, post-intervention study design to assess changes in clinical outcomes among adult patients. We will report on a retrospective medical record review of 104 program participants. One-year follow-up results of various clinical indicators (total serum cholesterol, low density lipoprotein cholesterol, high density lipoprotein cholesterol, triglyceride levels, glycated hemoglobin levels, fasting blood glucose, blood pressure, weight, and body mass index) will be reported. Analyses will determine changes in clinical outcomes of patients before and after nutrition intervention and document improvement (or lack of) in key biological measures and weight. A framework for future data collection, management, and evaluation of the nutrition counseling program will also be presented.
