HIV sentinel surveillance in Bolivia during 2000-2001: An early warning system for a low prevalence setting

During 2000-2001 the Ministry of Health implemented HIV sentinel surveillance in the four largest cities. Participating institutions included the National Laboratory (INLASA), the Social Security Maternity Hospital (La Paz), the Bolivian-Dutch Municipal Hospital (El Alto), the Bolivian-Japanese Technical Health School (Cochabamba), the National Tropical Disease Center (CENETROP) and Percy Boland Municipal Maternity Hospital (Santa Cruz). The United States Agency for International Development provided financial and technical support through Management Sciences for Health and the Integrated Health Project (PROSIN). During 2002-2004, activities will continue in the four cities, and will expand to border areas. Protocols followed WHO recommendations. The methodology for sample collection was anonymous-unlinked, minimizing participation and selection bias, and considering low HIV prevalence in Bolivia. Populations surveyed were pregnant women receiving antenatal care (Santa Cruz, La Paz), in-patients (El Alto), and food handlers (Cochabamba). Sample size was calculated using the Guaranteed Lot Quality Method. Samples were processed using ELISSA. External quality control was provided by INLASA (Cochabamba, La Paz, El Alto), CDC (INLASA), CENETROP (Santa Cruz), and the Hemocentro of Sao Paulo, Brazil (CENETROP). HIV seroprevalence in food handlers during 2001 was 0.13% (same as 2000). Seroprevalence in women attending the Maternity Hospital in Santa Cruz during 2001 was 0.87%, up from 0.00% in 2000. A 1997 study also found 0.00%. Seroprevalence among in-patients was 0.13%, down from 0.25% in 2000. In La Paz, seroprevalence among women during 2001 was 0.15%. In 2000 the three HIV-positives were all males; in 2001 all 10 positives were women of reproductive age.
