Using LQAS to Go to Scale with Child Survival Interventions in uThukela District, KwaZulu Natal, South Africa

METHODS: LQAS was used for supervising PHC activities in a pilot municipality, and in four expansion municipalities. Achievement of coverage targets was assessed with 24 interviews of client populations in each municipality. LQAS was used to determine whether the pilot municipality maintained coverage for MCH and HIV services from its end of project status in 1999. And whether “go-to-scale” municipalities achieved initial coverage targets.

RESULTS: The pilot area maintained the 1999 EOPS for 4 of 6 key indicators for the cohort of mothers of children 0-23 months (growth monitoring and promotion, knowledge of age of weaning, knowledge of pregnancy danger signs, knowledge of diarrhea case management). Two indicators fell below the EOPS (knowledge of feeding practices following a diarrhea episode, measles vaccination coverage). In the four expansion municipalities average coverage for key indicators were: GMP (67%), postpartum Vitamin A taken by mothers (49%), breastfeeding commenced within the first hour of birth (55%), knowledge of diarrhea case management (44%), knowledge that HIV can be transmitted through breast milk (75%), willingness to attend VCT (76%), knowledge of how HIV+ breastfeeding women can reduce risk to her child (13%). LQAS results indicated substantial variation among the municipalities for the first three indicators.

CONCLUSIONS: Results show how LQAS data were used to prioritize expansion municipalities by key indicators, and to determine whether the pilot municipality 1999 EOPS had deteriorated. Results demonstrate how to monitor a pilot area to maintain its coverage for key indicators, while emphasis is given to the new roll out areas.
