Experiences with the development, implementation, and evaluation of a weight management intervention for persons with type 2 diabetes

The goal of my practice-based initiative was to gain experience in the development, implementation, and evaluation of a state-of-the-art weight management intervention for persons with type 2 diabetes seen within two HRSA primary care facilities located in a rural, medically under-served community in South Carolina. Development of this CDC-funded project was carried out at the University of South Carolina Norman J. Arnold School of Public Health. My role with this project began in 1998 and continues to the present, during which time I progressed from study nutritionist, to lead interventionist, to my current role as project director. My experiences have provided opportunities for involvement with all aspects of this project. This experience complements the information I am learning in the classroom, while pursing my Master’s degree in Epidemiology. My willingness to see this project to completion has been challenging, but has allowed me to see how an initial idea progresses into a project being implemented in a community where it is needed. Several important outcomes resulted from this initiative which include: empowering individuals to improve their health by learning to control their diabetes, providing a service within primary care facilities that otherwise would not be available due to limited resources, and building partnerships with local agencies and primary care facilities to help bridge the gap between academia and the community.
