Community Health Workers: Assisting Vulnerable Adults in Achieving Self-sufficiency

The MOMobile, a program of the Maternity Care Coalition, is a community-based outreach and family support program. Located in one of the most vulnerable communities in Philadelphia, the North Philadelphia MOMobile provides specialized services with the goal of promoting each participant's optimal use of available public benefits and services in order to facilitate a successful transition to self-sufficiency. MOMobile Advocates reach out to isolated individuals through home visiting and ongoing telephone contact supporting participants by: 1. Assisting with navigation of time-limited opportunities for cash assistance and training; 2. Helping eligible working adults access non-cash income supports; 3. Linking participants to community services including those that assist with English proficiency and immigration issues; and 4. Supporting clients in addressing barriers to self-sufficiency and developing an achievable plan to overcome these barriers. This presentation will demonstrate the need for such an intervention, describe its components and present information regarding the impact of the program.
