Pathways © Outcomes-Based Community Care Coordination

Developed by the Community Health Access Project (CHAP), outcomes-based Pathways© provide accountability and improved outcomes to service providers and clients. Each Pathway clearly identifies a basic client need, provides evidence-based steps leading toward a positive resolution, and then only recognizes successful completion when the predetermined positive outcome is achieved. CHAP has implemented service reimbursement upon Pathway completion to ensure that clients reach a successful outcome before full payment is received from funding sources. Over 35 Pathways© address basic health and social service issues, including confirmed up-to-date immunizations for a child behind on shots and delivery of a healthy newborn for an at-risk pregnant mother. In the first year of implementation, over 4000 Pathways have been completed, indicating documented client successes. Fully committed to the Pathways’ approach, CHAP has redesigned its training, database, and employee productivity to focus on the new model. CHAP’s Pathways© model is the basis for several notable state and national grant awards, including the top 2001 American Academy of Pediatrics’ Community Access to Children’s Health (CATCH) grant. Pathways© are also an integral part of the federal Community Access Program in partnership with the Ohio Department of Health and five Ohio programs to demonstrate a community care coordination model that includes CHAP’s college-credit training, seven Pathways, and an integrated outcomes-based data system. CHAP maintains three culturally diverse projects in Ohio and offers support for other CCC programs through the CHAP Training Institute and Strategic Outcomes Partnerships.
