This session will examine the population impact of longitudinal changes in the hospital infrastructure on the East Coast and in California. It will provide an important opportunity to assess systemwide differences and similarities of hospital infrastructure changes in major population centers. For the first time, longitudinal changes in the hospital infrastructure will be compared in the same session by two research teams working in different areas of the US. The teams will report on and compare absolute hospital survival as well as hospital infrastructure changes (beds, nursing staff, emergency rooms, pharmacies). The focus will be on hospitals serving vulnerable populations, e.g., inner city and rural, poor or uninsured, and pediatric patients. The longitudinal impact of changing public policies (prospective payment, managed care, employer crowdout) will be evaluated for their impact on population-based measures of patient access, safe staffing, and quality of care after admission.
Learning Objectives: 1. Describe longitudinal changes in hospital survival in the Eastern US and in California. 2. Compare the impact of the changing healthcare environment on survival of hospitals caring for vulnerable populations. 3. Evaluate the impact of hospital infrastructure changes on quality of care.
