| Since 1995, 25 public-private partnerships nationwide—called "Community Care Networks"—have been addressing the challenge of improving community health by bringing together providers, public health, social services, government, education, and other institutions. After a brief description of the Community Care Network vision, presenters will share how these partnerships addressed questions of health and quality of life in their very different communities by listening to the community and involving residents in decision making and program implementation. The session will also discuss how a theory of action (logic model) approach to strategic planning and evaluation helped the partnerships think through choices about initiatives, short-term objectives, and long-term goals. Representatives from two communities—one very rural county and one inner-city neighborhood—will present in-depth information on their partnerships. The focus will be on how the partnerships enabled health professionals to enhance services and achieve more than any one institution could have accomplished alone | |||
