Use of a PHN Assessment Tool for data collection and analyze of population health needs based on Healthy People 2010 indicators

This presentation will provide participants with information about the recently developed PHN Assessment Tool which is an integral part of documenting PHN practice in Los Angeles County. The tool development will be described. The use of the tool in the generalist PHN practice in Los Angeles County will be discussed. The relationship of the tool content to assessment of the Healthy People 2010 leading health indicators will be explained and demonstrated. Data for Los Angeles County for 2002 gathered through the use of this tool will be presented and the data will be discussed in the context of its contribution to an overall population based assessment of community health needs. In Los Angeles County this data has become an important part of public health surveillance and informs decisions about the directing of scarce public health nursing resources to particular community health needs.
