This session presents the process of developing and sustaining a successful collaboration between public health practice and nursing education. This collaboration between Nobles-Rock Public Health Agency and South Dakota State University (SDSU), College of Nursing provides rich clinical projects in Southwestern Minnesota. The collaboration was recognized by the Minnesota Department of Health as a Prairie Model for Public Health Nursing Clinical which was an outcome of the Linking Public Health Nursing Practice and Nursing Education to Promote Population Health HRSA Grant. Because of the increased numbers of nursing students enrolling in the nursing program and the shortage of public health nurses, this collaborative model has been adopted by an adjacent agency in order to provide additional clinical sites. Another purpose of this collaborative project is to recruit baccalaureate students into public health as well as to provide population-based interventions by the agency. The Nobles-Rock director and the SDSU instructor mentored a second agency director and public health staff in the process of collaboration. The outcome of this collaborative effort is the duplication of a public health project by the collaborating agencies. Agency preceptors, nursing instructors and nursing students develop Population-based Interventions. The target population for these projects includes day care providers, low income families, business and industry. Projects developed by agency staff and students are based on the Minnesota Intervention Wheel.

Learning Objectives:

• 1.Discuss model that was used to promote link between Public Health Nursings and Nursing Education in a 6 county area in Southwest Minnesota and Eastern South Dakota.
• 2.Articulate mentoring by agency director and nursing educator of adjacent agency staff which resulted in development of new clinical site.
• 3.Describe 2 student/agency projects which were developed and implemented by linking partners.
