Faith and health leaders collaborating to eliminate health disparities

The principle mission of the Institutes for Public Health and Faith Collaborations, funded by the Centers for Disease Control and Prevention, is to develop leadership that leads to community transformation for the elimination of health disparities. During 2002 and 2003, three 4-day institutes have been held generating 34 teams from around the U.S. who have developed and are implementing collaborative action plans in their respective communities. Each team is comprised of leaders representing both faith-based as well as health organizations. This session will have representatives from four of the alumni teams share a summary of their action plans and most importantly, address lessons learned from their collaborative activities. In addition an evaluation summary of the short and long term outcome preliminary findings for the entire project will be reported.
