AAPCHO’s BALANCE Program for Diabetes funded six (6) community health centers to implement culturally and linguistically appropriate diabetes activities for AAPI communities. In order to measure the progress towards long-term impacts targeted by the program goals, BALANCE developed a program logic model. Each project site worked with the BALANCE program to develop an extensive evaluation plan in relationship to the BALANCE logic model, including a baseline data collection instrument. The evaluation plans included work plans identifying the interventions, target population, outcomes, and evaluation/evidence of change for each planned activity. Program evaluation plans included process and outcome measures for community/patient education, collaboration with other community-based organizations and media activities. In order to conduct data collection, problem-solve, and to share and compare data across project sites, common outcome measures, instruments and definitions were identified and utilized across the six community health centers.
