Web-based health information and educational materials that target behavioral change are available for many diseases and health conditions. Similarly, these materials can also be found on various web sites that focus on asthma and other respiratory diseases. However, few, if any, of these sites target personnel from the public agency perspective. The Creating a Medical Home for Asthma (CMHA) program aims at healthcare providers in public health settings. As part of an effort to translate the intervention protocol from research to practice, the process also involved implementing a method for its widespread dissemination. Based on this experience, we present: (1) a framework for collaborative activities with both the developer and distributor for the CMHA materials; (2) an interactive demonstration of the various components of the web-based materials; and (3) a discussion of critical points for consideration when attempting to implement and evaluate such a web-based program. Following the presentation of our framework that includes translation, evaluation, education, and dissemination, we will take the audience through a demonstration of the CMHA web site to show how to use and adapt the information when viewing on-screen and after downloading it. Lastly, we will lead a discussion among the collaborators to highlight the challenges that we faced in implementing this technology and offer key concepts for consideration in the evaluation of this web-based program for healthcare providers.
