Compliance with California smoke-free bar policy: Bartender-patron dynamics

A smoking ban in California worksites was extended to bars at the outset of 1998. In this study, we evaluated the effectiveness of the ban through several methods, including sending pairs of observers to 121 randomly selected standalone bars in San Francisco on four different occasions, recording structured observational checklists on handheld computers and writing structured narrative field notes. Additionally, 60 semi-structured interviews were conducted with staff and patrons from these bars. This presentation focuses on the social dynamics between bartenders and their patrons, and how these dynamics are related to the prevalence of smoking in some of these bars, in defiance of the statewide smoke-free bar policy. The structured observations were downloaded to a database for statistical analysis, and the structured observer narratives and the interview transcripts were compiled in ATLAS.ti, software, facilitating text analysis. Analysis of 481 observations indicated that employee smoking was highly correlated with patron smoking (c2=107.52, p<.0001). Observations and interviews shed light on bartender – patron dynamics that contribute to compliance or noncompliance with the bar smoking ban. Identified themes included role modeling and power wielded by bartenders on duty and the desire of patrons to please the person in authority. Theoretically, these dynamics are tied to compliance or noncompliance in four ways: through (1) power relations, (2) role modeling, (3) social control and (4) social networks of smokers and nonsmokers. Study implications include focusing on the bartender-patron relationship when designing smoke-free bar policies.
