Objectives: A nationwide campaign on health promotion entitled "Healthy Japan 21" was commenced in 2000 by the Japanese government. Smoking is one of the most important measures of this campaign. This paper describes the present status of the implementation of the measures against smoking in the local government offices in Japan. Methods: A questionnaire form was mailed to the section chiefs of the health centers of all the local government offices (n=3251) in January 2001. The form included questions on various measures against smoking in the governmental office. Results: 2791 local government offices responded (response rate: 85.9%). The measures of "education on the health effects of smoking" and "separation of smoking" were implemented in 81.0% and 80.7%, respectively. No specific measures against smoking were implemented in 5.0% of the local government offices. Among governments with "education on the health effects of smoking", 76.3% adopted poster notice and 41.0% did distribution of printed materials. Among governments with "separation of smoking", 78.9 % of the offices adopted area separation and 32.3% did time separation. Detailed description will be presented. Conclusion: The present status of the implementation of the measures against smoking in local government in Japan was summarized. Majority of the governments adopted "education on the health effects of smoking" and "separation of smoking in the office". It seems desirable to adopt more effective and active measures for attaining healthy environment.
