Answering the community call, part IV: Daily operations and management of the team approach to health care

The right people can make all the difference in the success of your program. And, the organization of the daily, weekly, monthly responsibilities of those team members will streamline healthcare delivery. Successful management and coordination of these team members and their responsibilities within the team is key to running an effective project. Weekly and monthly task assignment within the team is carefully monitored through monthly "core" team meetings. Within these meetings is the time to discuss success and those program sections that need improvement. Team members take this opportunity to voice their opinions and suggestions on improvements or the addition of new concepts. The development and implementation of data collection and charting tools is necessary to maintain accurate and measurable information, as well as the design and availability of educational materials relating to your team's specific health issue. These tools can include: · health issue advise brochures for clients and service providers, · resource and/or referral guide, · data collection tools, · and evaluation tools In this session obtain documentation and protocol tools. Speak with members currently involved in a community-driven health care team model.
