Kaiser Permanente, Care Management Institute’s diabetes care program: Population based disease management within an integrated delivery system

Kaiser Permanente's Integrated Diabetes Care Program was the first in a portfolio of high-priority care management programs developed by Kaiser Permanente's Care Management Institute (CMI). The vision of CMI's Integrated Diabetes Care program is to improve the lives of all of KP's members with diabetes by facilitating the delivery of the highest quality care within all of KP's regions. KP's collective diabetes management effort strives to be a national model for delivering effective, consistent, evidence based, personalized, and compassionate care for patients with diabetes.

Diabetes affects a large number of Kaiser Permanente members, approximately 7.6 percent of KP's adult membership. KP serves over 422,000 adults with diabetes in 29 medical centers and 423 medical offices located in California, Colorado, Georgia, Hawaii, Maryland, Ohio, Oregon, Virginia, Washington, and the District of Columbia.

The overriding objective of CMI's diabetes care program is to enhance the health of KP members with diabetes, control disease progression, and prevent disability. The program is strongly grounded in medical evidence and extends to many aspects of our comprehensive care management program, including: national, biennially-reviewed, evidence-based clinical practice guidelines; models of care (e.g. patient registries, stratification methodologies, commitment to a team approach and evaluation of care); clinician resource materials; patient resource materials; a measurement system and National Outcome Reports; implementation support, including technology tools; regional calls and forums to disseminate new programs and share best practices throughout KP's regions.
